Advanced CKD Patient Management Toolkit

RPA developed the Advanced CKD Patient Management Toolkit through a rigorous scientific process to assist practitioners with implementing the recommendations included in RPA’s third evidence-based clinical practice guideline, Appropriate Patient Preparation for Renal Therapy (RRT). The toolkit is organized in four main areas – assess, tailor, implement and evaluate - includes 16 categories and sets of tools for healthcare providers and kidney patients.
